Some young people in school have transition planning meetings.
What are transition planning meetings?

They are meetings to help you plan your future

They look at the support you get in school and if this is helping you

They are arranged by the school once a year

The school invites other people into the meetings
Who comes to these meetings?
The school will invite people to the meeting. This will include:

You

A Teacher

Your parents / carers
It could also include other people who work with you such as:

Your Careers Adviser

Your Social Worker

Other people who can help you
What happens in the meetings?
The meeting is all about you and your future.

If you want to, you can talk about what you think is going well and things that aren't going as well

You will be able to tell people what you want to do in the future

Other people may talk about how they can help you

Everyone will agree a plan for you to know what you need to do and what other people will do to help you
What do I need to do at the meetings?
You don’t have to do anything if you don’t want to but it’s important that people know what you want.
If you don’t want to speak in the meeting talk to your Teachers or someone else before the meeting and tell them what you would like people to know.
If you do want to speak, you could tell people:
- How things are going in school for you
- If you need more help with something
- What you would like to do in the future and ask who can help you
